Please include with your referral
- Patient demographics and contact information
- Insurance information (front and back of card)
- Reason for referral and relevant clinical notes
- Recent labs: creatinine, eGFR, CMP/BMP, urinalysis, urine protein if available
- Recent renal imaging reports (ultrasound/CT), if available
- Current medication list

- Elevated creatinine or declining eGFR (CKD stages 1–5)
- Proteinuria or hematuria
- Resistant or secondary hypertension
- Electrolyte disorders (hyperkalemia, hyponatremia)
