1 What does this test mean?

The Hemoglobin A1c test measures your average blood glucose (sugar) over the last 3 months. This test, along with your home blood glucose testing, can give you a better idea of how well your diabetes is controlled.

Hemoglobin is part of a red blood cell. It carries oxygen throughout your body. Glucose (sugar) in your blood sticks to hemoglobin, where it remains for the life of the red blood cell. The combination of hemoglobin and glucose is called Hemoglobin A1c (HbA1c). This test measures the percentage of hemoglobin that has glucose attached to it. Because red blood cells live for about three months, this test reflects your glucose level over the last three months.

2 How is this test done?

The test is done in the laboratory using a routine blood sample. It is not a fasting test, so you can have it done at any time of the day. Results will take 2 to 3 days.

3 What is the goal of the HbA1c test?

The goal of the Hemoglobin A1c test for most people with diabetes is: To achieve a level of less than 7% (< 7.0%). The American Diabetes Association (ADA) recommendation establishes that A1c levels of 6.5% and above call for a diagnosis of diabetes.

If you can lower your Hemoglobin A1c numbers by any amount, you will improve your chances of staying healthy and reduce your chances of getting kidney disease, eye damage, or nerve damage. A change in treatment is usually needed when the Hemoglobin A1c is over 8% (> 8.0%).

Nephrology Considerations in Chronic Kidney Disease (CKD)

In patients with advanced chronic kidney disease (Stages 3–5) or those on dialysis, red blood cell survival is often shortened. Furthermore, therapy with erythropoiesis-stimulating agents (ESAs) and intravenous iron can cause falsely lower HbA1c readings.

KDIGO clinical practice guidelines recommend individualized glycemic targets (typically ranging from 6.5% to 8.0%) to balance microvascular renal protection against the life-threatening risks of hypoglycemia. Always consult Dr. Masood or Dr. Siddiqui for your specific clinical target.
